The 1095-C forms for tax year 2025 have now been uploaded to the State Health Plan website. The 1095-C Form is the information form that reports the minimum essential health coverage we provided to you for the 2025 tax year.We are not mailing these forms to active employees this year. You will need to follow the attached directions to download them electronically from the State Health Plan website.If you have any questions or issues with your 1095-C Form, please contact Leslie Williams at lewilliams@caldwellschools.
